This reusable Novolog® Mix 70/30 FlexPen coupon card been has been widely distributed by Novo Nordisk sales representatives beginning October, 2009. This offer is slated to expire 6/26/2010 (although Novo usually honors coupons and rebates past the expiration date).

These coupons are part of a comprehensive patient support program designed to improve the likelihood of a successful outcome for diabetics taking Novolog® Mix 70/30 FlexPen.

If your doctor does not have any of these coupons on hand, he should call his Novo Nordisk representative who handles Novolog® Mix 70/30 FlexPen. If he does not know who his representative is he should call Novo Nordisk at (609) 987-5800. They will send a representative to drop off the coupons and explain how the program works.

This program is only appropriate for doctors who normally prescribe insulin (internists, primary care doctors, endocrinologists). Not all doctors qualify for this program. Pharmacists may not request these coupons.

How It Works:
After you obtain the card, call 1-866-829-4370 to activate the Novolog® Mix 70/30 FlexPen coupon card. Once activated, the card entitles you to $50 off your first prescription and $25 off your next three refills. (Only new patients qualify for the one-time $50 discount. If you are already a Novolog® Mix 70/30 FlexPen user, then this card entitles you to four $25 discounts).

- The amount of the discount can not exceed your out-of-pocket costs.
